Daviess Community Hospital Launches 2025 Community Health Needs Survey

Daviess Community Hospital has launched its 2025 Community Health Needs Assessment survey and is encouraging residents throughout southwestern Indiana to participate.

DCH completes an assessment every three years to ensure its programs, services and outreach reflect the priorities of the people it serves.

The online survey is available in English, Spanish and Haitian Creole at dchosp.co/chna25 and is open through July 31st.

Paper copies are available at the hospital’s main‑lobby front desk.

Responses are anonymous and will be analyzed to create DCH’s next Community Health Needs Assessment Report.
